Requirements for Enrollment:

1. Form 138 (High School Card) or Transcript of Records
2. Good Moral Certificate or Brgy. Clearance
3. NSO Birth Certificate (photocopy)
4. 2 copies of ID picture (passport size) – White background / with collar
5. Youth Profiling for Starring Career(YP4SC)
